Calcific aortic valve stenosis is the most common valvular disease in developed countries, and the major reason for operative valve replacement. In the US, the current annual cost of this surgery is approximately 1 billion dollars. Despite increasing morbidity and mortality, little is known of the cellular basis of the calcifications, which occur in high-perfusion zones of the heart. The case is presented of a patient with calcific aortic valve stenosis and colonies of progressively mineralized nanobacteria in the fibrocalcific nodules of the aortic cusps, as revealed by transmission electron microscopy. Consistent with their outstanding bioadhesivity, nanobacteria might serve as causative agents in the development of calcific aortic valve stenosis.

The case is reported of a 36-year-old male patient suffering from congenital pulmonary stenosis who previously had undergone pulmonary balloon valvuloplasty. During the past nine years, he had experienced recurrent attacks of rheumatic fever that gradually damaged all four heart valves. The patient underwent aortic, mitral and pulmonary valve replacement with tricuspid valve annuloplasty and pulmonary artery reconstruction. Histologically, all heart valves--including the pulmonary--had similar changes that corresponded to chronic rheumatic disease.

The case is reported of a patient who underwent cardiac surgery for pulmonary valve stenosis as a child, and presented as an adult with signs and symptoms of severe congestive heart failure. The left ventricle showed an increased trabecular pattern in the region of the apex, the mitral annulus was severely dilated with mitral incompetence, the right ventricular out-flow tract (RVOT) was largely dilated with aneurysm of both pulmonary arteries, and there was evidence of pulmonary valve incompetence. Previously, rare cases have been reported of persistent left ventricular non-compaction in patients with congenital left or RVOT obstruction. Non-compaction of the ventricular myocardium is an inherited autosomal dominant disorder; to date, four genes and one genetic locus have been found to be associated with non-compacted ventricular myocardium. The condition is characterized by arrhythmias, thromboembolic events and heart failure, but affected individuals may not be symptomatic. The present case represented a strange association between non-compacted left ventricle, mitral annular dilation with persistence of a normal leaflet and subvalvular mitral valve apparatus, and RVOT dilation with pulmonary artery aneurysms.

Pericardial stentless valve prostheses have shown excellent results in aortic valve replacement. However, due to flexibility of the prosthetic material, immobilization and subsequent partial valve thrombosis may occur in case of non-symmetric sinuses of Valsalva. This report details an implantation technique designed to prevent stress on the non-coronary cusp of pericardial stentless prostheses, thereby avoiding this problem.

Background and aim of the study: The CarboMedics Top Hat supra-annular aortic valve allows a one-size (and often two-size) increase over the standard intra-annular valve. This advantage should minimize the risk of patient-prosthesis mismatch, where the effective prosthetic valve orifice area is less than that of a normal valve. It is suggested that the ability to implant Top Hat valves having greater size, relative to standard intra-annular valves, may currently be under-utilized. Further, there has been some concern that Top Hat implantation can cause obstruction of the coronary ostia. This study evaluates the authors' clinical experience with Top Hat supra-annular aortic valve size selection, and the technical aspects of implantation.

Methods: Between January 1999 and October 2005, a total of 251 consecutive patients underwent 252 aortic valve replacements with Top Hat supra-annular mechanical prostheses (CarboMedics, Inc., Arvada, CO, USA) at two institutions. Size frequency distribution was compared to published series, and to the manufacturer's US registry. The ventriculoaortic junction (VAJ) size was available in 234 patients, and compared to the size of the Top Hat valve implanted.

Results: Valves implanted were 2 mm larger than the VAJ in 56% of patients (130/234), 3 mm larger in 1.7% (4/234), and 4 mm larger in 42% (98/234). Numbers of 19-mm and 21-mm valves were less (p <0.001) in the present series (22%) than in published series (52%) and the US registry (46%). No patient required unplanned coronary bypass, and 30-day mortality was 2.0% (5/251), indicating a good safety profile for the valves implanted in this series.

Conclusion: The general distribution of implant sizes in the US indicates that cardiac surgeons may be under-sizing the Top Hat supra-annular aortic valve during aortic valve replacement, thereby missing the full advantage of its unique design for lowering the transvalvular gradient and, consequently, left ventricular workload, leading to a reversal of left ventricular hypertrophy.

Background and aim of the study: Although, in recent years, atrial fibrillation (AF) ablation has become an effective concomitant procedure in cardiac surgery, it is unclear whether the outcome of the procedure is determined by the underlying valve disease.

Methods: Between 2003 and 2005, 191 patients (100 females, 91 males; mean age 70+/-8.7 years) underwent concomitant left atrial (LA) ablation. Among these patients, those with permanent AF (pAF) and mitral (MVD; n = 64), aortic (AVD; n = 37), and combined valve disease (CVD; n = 23) were prospectively studied after three, six and 12 months, and annually thereafter. The predictive values of preoperative variables for postoperative AF were examined.

Results: AVD patients were older than MVD patients (74 +/-7.8 versus 66+/-8.6 years; p <0.001), and presented smaller atria (48+/- 5.7 versus 53+/-8.0 mm; p <0.05), but CVD patients were similar to MVD patients in terms of these parameters (age 70+/-9.3 years, LA diameter 54+/-9.6 mm). Ablation caused no injury or death in any of the patients. Within 30 days after surgery, three (4.7%), three (8.1%) and two (8.7%) of the MVD, AVD and CVD patients, respectively, had died (6.4% overall mortality). The sinus rhythm (SR) conversion rate was 76.8, 63.3 and 58.8% (p = NS) after a mean follow up (FU) of 10+/-4.0, 9+/-4.2 and 10+/-3.9 months (p = NS) in the MVD, AVD and CVD groups, respectively. FU was 97% complete. During FU, four (6.6%), two (5.9%) and four (19.0%) MVD, AVD and CVD patients died, respectively. Univariate analysis demonstrated a shorter AF duration (42+/-49.0 versus 85+/-67.2 months; p <0.05) and smaller LA diameter (50+/-8.1 versus 55+/-9.2 mm; p <0.05) in patients with SR versus non-SR. Multivariate analysis revealed AF duration (Odds ratio (OR) 1.01, 95% CI 1.00-1.02, p <0.05) and LA diameter (OR 1.08, 95% CI 1.01-1.15, p <0.05) as independent predictors of SR conversion. Overall, 86% of patients with two or less years' duration of pAF were in SR at FU.

Conclusion: AF duration and LA diameter, but not the type of valve disease, predict SR conversion after concomitant ablation of pAF in valvular heart surgery.

Background and aim of the study: Little is known of the local role of nitric oxide (NO) in heart valves in relation to heart valve diseases. The study aim was to examine NO release and the expression of both endothelial nitric oxide synthase (eNOS) and inducible nitric oxide synthase (iNOS) in relation to early local changes in porcine mitral valves.

Methods: A histological evaluation of mitral valve leaflets from slaughter pigs and sows was made, and the expression of eNOS and iNOS protein measured using immunohistochemistry. Furthermore, mRNA levels of eNOS and iNOS were measured using real-time RT-PCR. A calibrated NO-specific electrode was used to measure local NO release in specific regions of the anterior mitral leaflet from slaughter pigs and sows interchordally at the tip of the leaflet (region A), at the chordal insertion (region B), and at the center of the leaflet (region C).

Results: Leaflets from sows had an increased accumulation of mucopolysaccharides (MPS) compared to those from slaughter pigs. Furthermore, mRNA levels of eNOS and iNOS were significantly increased in region C due to very high levels of expression in some sow leaflets. NO release in the sow mitral valve leaflet was increased in regions B and C compared to that in region A.

Conclusion: The relative distribution of NO release is increased in regions of porcine mitral valve leaflets with deposition of MPS and defraction of the valve structure, which may reflect changes in both eNOS and iNOS expression.

Background and aim of the study: Human mesenchymal stem cells (MSCs) are a potential cell source for the tissue engineering of biological structures, including cardiac valves. A comprehensive, phenotypic analysis of MSCs and, for the latter, their comparison with valve interstitial cells (ICs) is therefore essential.

Methods: Isolates of bone marrow-derived human MSCs and human cardiac valve ICs were extensively phenotyped for their expression of membrane proteins involved in adhesion and cell-cell communication, cytoskeletal components, extracellular matrix (ECM) proteins and gene expression of WNT/FZD/SFRP/DKK/LRP family members.

Results: MSCs and valve ICs (>80%) expressed fibroblast surface antigen, smooth muscle alpha-actin, vimentin and CD44; expression of MHC class I and II and calponin was inconsistent, and a small proportion expressed desmin and smooth muscle myosin. CD105 was weakly expressed by a low percentage of valve ICs (<10%) compared to MSCs (>90%). ECM components made by both cell types demonstrated similar levels and patterns of staining, although expression of elastin was not detected by both cell types. Adhesion molecule expression was highly variable among the MSC isolates and between the two cell types, with the predominant integrins being alphal, alpha3, alpha5, and beta1 by both cell types. PCR analysis of WNT/FZD/SFRP/LRP family members revealed a greater range of the WNT family of genes being expressed in MSCs compared to ICs.

Conclusion: The study results provided an extensive fingerprint of valve ICs and of MSCs for the tissue engineering of biological structures and for the manipulation of their desired phenotype. MSCs represent a promising cell type for valve tissue engineering, and will require extensive phenotyping after differentiation.

Background and aim of the study: The new Freedom SOLO aortic valve, as a modification of the Pericarbon Freedom stentless valve, requires only one suture line. The study aim was to compare both prostheses with regard to surgical convenience and early postoperative hemodynamics.

Methods: In this case-matched study, 90 patients underwent primary elective isolated or combined aortic valve replacement (AVR) between 2001 and 2004. The Freedom SOLO valve was implanted in 30 patients using a supra-annular, subcoronary technique with one continuous subcoronary suture line (group FS). For comparison, the Pericarbon Freedom valve was implanted in 30 patients using an interrupted suture (group PFI), and in 30 patients using a continuous suture line at the inflow site (group PFC). Patient hemodynamics at discharge were investigated by monitoring echocardiographic peak and mean gradients, and regurgitation.

Results: The mean extracorporeal circulation time (75.5+/-22.7 min in FS; 83.7+/-21.0 min in PFC versus 110.9+/-31.4 min in PFI) and cross-clamp time (56.0+/-18.6 min in FS; 62.2+/-16.2 min in PFC versus 87.7 +/-25.4 min in PFI) were significantly shorter with the continuous implantation techniques. Hemodynamics were most favorable for the Freedom SOLO valve as compared to either implantation technique of the Pericarbon Freedom valve (mean gradient 7.4+/-4.6 mmHg in FS versus 10.6+/-6.0 mmHg in PFC and 11.0+/-5.7 mmHg in PFI). There were no paravalvular leakages or transvalvular regurgitations.

Conclusion: The Freedom SOLO stentless valve is easy to implant and demonstrates superior early postoperative hemodynamics. Due to the reduced cross-clamp time, it also offers an attractive option in isolated and combined AVR.

Background and aim of the study: Presumed benefits from stentless bioprostheses include larger orifice areas with lower transvalvular gradients, and improved hemodynamic flow characteristics and annular mechanics. Herein are reported the results of a large series of the Sorin Pericarbon Freedom stentless valve implanted in the aortic position.

Methods: Between July 1998 and June 2003, a total of 102 consecutive patients (58 males, 44 females; mean age 71.7+/-7.8 years; range: 28-87 years) requiring aortic valve replacement (AVR), including those undergoing concomitant procedures of coronary artery bypass, mitral valve repair and mini-maze, was recruited. The predominant lesion was aortic stenosis (n = 92; 90.2%); aortic regurgitation (AR) occurred in five patients (4.9%) (including three with endocarditis), and mixed aortic valve disease in five (4.9%). Six patients (5.9%) had undergone previous cardiac surgery. The median preoperative NYHA class was III. Thirsty-six patients (35.3%) underwent AVR alone. The mean valve size was 25 mm (range: 21-29 mm). Sixty-three patients (61.8%) had concomitant coronary artery disease that required a mean of 2.4+/-1.1 bypass grafts; three patients (3.0%) had combined AVR and mitral valve repair. The study end points observed were mortality, valve failure due to degeneration or endocarditis, reoperation, thromboembolism, transvalvular gradients and left ventricular (LV) mass regression.

Results: The median follow up for all patients was 31 months (range: 12 months to 5 years). The mean total cross-clamp time was 71.7+/-17.6 min without associated procedures, and 93.9+/-19.7 min with concomitant procedures. Early mortality was 4/102 (3.8%); actuarial survival over five years was 89.2%. Freedom from thromboembolism over five years was 95.9%, from reoperation 100%, and from endocarditis 99.98%. Fifty-seven patients (56.4%) had no AR detected postoperatively, and 34 (33.3%) had trivial or mild AR. A significant decline was observed in indexed LV mass regression within six months of surgery, from 190+/-72 g/m(2) at baseline to 152+/-47 g/m(2) (p = 0.01).

Conclusion: In an elderly population with a high incidence of coronary artery disease, the Sorin Pericarbon Freedom stentless valve offers excellent hemodynamics, resulting in significant regression of left ventricular hypertrophy, together with acceptable operation times, morbidity and mortality in the medium term.

Background and aim of the study: The Silzone-coated St. Jude Medical valve was developed to reduce prosthetic valve endocarditis, but in the Artificial Valve Endocarditis Trial (AVERT) was recalled following reports of major paravalvular leaks. A separate study suggested an increased risk of thromboembolic complications associated with the Silzone valve. Herein is reviewed the present authors' experience in patients with Silzone valves.

Methods: Between April 1998 and November 1999, 46 patients (28 males, 18 females; mean age 62.6 years; range: 41-78 years) received a total of 49 Silzone valves. Seven of the patients were in NYHA class IV, and 29 in class III; four patients had active endocarditis. Twenty-one patients underwent aortic valve replacement, 22 mitral valve replacement, and three had both mitral and aortic valve replacement. Concomitant procedures were performed in nine patients.

Results: The 30-day mortality was 6.5 % (n = 3). At post-mortem examination, the valves were found to be seated well, with no evidence of malfunction, thrombotic occlusion or infection. Follow up examinations were conducted initially at six months after surgery, and annually thereafter. All patients underwent transthoracic echocardiography. Follow up was 100% complete; the mean follow up period was 73.5 months (range: 62-81 months). Six late deaths have occurred; five deaths were due to non-cardiac causes, and one cardiac death was unrelated to the valve prosthesis. No cases of paravalvular leak have been identified. Two patients had postoperative thromboembolic complications, but echocardiography did not demonstrate the presence of thrombus or paravalvular leakage.

Conclusion: In this single-center, non-randomized study of 46 patients, the findings of increased paravalvular leak with the Silzone valve, as identified in AVERT study, could not be confirmed. Moreover, the incidence of thromboembolic complications reported was modest.

The management of aortic valve disease in children and young adults is often a controversial and complex problem. The Ross operation has become an important option for aortic valve replacement in patients with both congenital and acquired disease of the left ventricular outflow tract (LVOT). The acceptance of this procedure has been slow because of the technical demands of the operation and the inherent need for reconstruction of the right ventricular out-flow tract (RVOT), thereby placing two valves at risk. Although more than three decades have passed since the Ross operation was first described, it is only during the past 10 years that it has been increasingly considered for pediatric patients with a wide spectrum of congenital abnormalities of the LVOT. The advantages of the autograft valve include growth potential, optimal hemodynamic performance, and freedom from anticoagulation and hemolysis. However, concerns regarding potential dilatation of the autograft, reoperation for autograft dysfunction, and replacement of RVOT conduits persist. Further, doubts have been expressed by others regarding the indications for the Ross operation in patients with a bicuspid aortic valve and with aortic valve insufficiency. This review evaluates existing evidence on the safety and efficacy of Ross procedure for pediatric and adolescent patients.

Background and aim of the study: Concomitant replacement of the mildly dilated ascending aorta during aortic valve replacement (AVR) is controversial because progress of aortic dilatation is uncertain after elimination of the valvular lesion. The study aim was to determine factors influencing the aortic dimensions, to analyze their clinical impact, and to identify criteria for concomitant surgery on the aorta.

Methods: Between February 1994 and May 1999, 100 patients with tricuspid aortic valve disease received the stented porcine Mosaic prosthesis within a FDA approval study. Follow up (mean 4.8 years; range: 0.1-8.8 years; total 483.4 patient-years) included documentation of adverse events and transthoracic echocardiography with measurements of the aortic sinus, sinotubular junction, and ascending aorta.

Results: Baseline aortic dimensions were dependent on gender and body surface area, but independent of the type and extent of valvular lesion, patient age, and atherosclerotic risk factors. Larger baseline aortic diameters were associated with smaller postoperative annual aortic expansion rates (r = -0.47, p <0.001). In patients with baseline aortic dilatation > or =40 mm (10.2%; mean 42.5+/-2.6 mm), aortic diameter decreased during follow up (p = 0.032; expansion rate -1.9+/-2.0 mm/year). Baseline aortic dilatation did not influence postoperative morbidity and mortality. Prosthetic regurgitation was associated with increases in aortic diameter (p <0.001). Survival was reduced in patients with aortic expansion rates >3.6 mm/year (0.0% versus 68.2+/-9.7%; p <0.001).

Conclusion: AVR without concomitant surgery on the aorta in patients with mild aortic dilatation is feasible, as aortic diameters were decreased after removal of the diseased valve. The aortic expansion rate had a strong prognostic importance, even on aortic diameters, which are considered to be within normal ranges.

Background and aim of the study: Limited data are available on the effect of percutaneous balloon mitral valvulotomy (PBMV) on quality of life (QoL) in patients with mitral stenosis (MS), and whether the effect is similar between patients in sinus rhythm (SR) and with atrial fibrillation (AF). The study aim was to determine the effect of PBMV on the QoL of such patient groups.

Methods: A total of 130 patients with symptomatic MS and scheduled for PBMV was studied. Patients with AF were predominantly male, and had more severe symptoms, a larger left atrial size and a lower left ventricular ejection fraction (LVEF). Baseline characteristics were collected together with PBMV outcomes. QoL was assessed using the SF-36 questionnaire at baseline, and at one and six months after PBMV. The QoL assessment included eight aspects: physical functioning; role physical; bodily pain; general health; vitality; social functioning; role emotional; and mental health. The raw scale was transformed into a transformed scale from 0 (worst) to 100 (best). Physical and mental subscales were calculated.

Results: The cardiac rhythm was AF in 65 patients (50%). QoL on physical and mental scales was significantly improved after PBMV. There was no difference in the effect of PBMV on QoL improvement in patients with AF and SR. QoL improvement was demonstrated in all aspects, except for bodily pain. PBMV also improved NYHA functional class and mitral valve area, but decreased left atrial diameter and right ventricular systolic pressure in both groups. An increase in LVEF was observed in patients with AF CONCLUSION: PBMV was shown to improve QoL in patients with MS, irrespective of their baseline cardiac rhythm.

Background and aim of the study: In patients with mitral regurgitation (MR) due to degenerative mitral valve prolapse (MVP), preoperative atrial fibrillation (AF) has been identified as an independent predictor of survival after surgery for MR. Thus, the determinants of preoperative AF may have critical implications to evaluate the timing of mitral valve repair. The study aim was to investigate the role of left atrial (LA) volume in predicting preoperative AF in patients with severe MR due to degenerative MVP.

Methods: Sixty-six patients with severe degenerative MR (regurgitant volume > or =60 ml, regurgitant fraction > or =50%, effective regurgitant orifice area > or =0.4 cm(2)) in sinus rhythm (SR) at diagnosis and conservatively managed were eligible for the study. Complete two-dimensional (2-D) echocardiographic and Doppler measurements, including the measurement of maximum LA volume, were performed in all patients.

Results: During follow up under conservative management (18.1+/-4.8 months), eight patients (12%) experienced conversion to AF, and 58 remained in SR. The mean LA dimension was 4.0+/-0.5 cm in patients with SR, and 5.1+/-0.8 cm in those who developed AF (p <0.0001). The mean LA volume and LA volume index (indexed to body surface area) were 95 +/-23 ml and 60+/-14 ml/m(2) respectively in patients with SR, and 166+/-66 ml and 104+/-42 ml/m(2) respectively in those who developed AF (both p <0.0001). The optimal cut-off value for LA volume to predict AF conversion was 117.5 ml (sensitivity 88%, specificity 83%), and for LA volume index was 75 ml/m(2) (sensitivity 88%, specificity 88%).

Conclusion: LA volume measurement should be considered in patients with degenerative severe MR diagnosed in SR. A LA volume index > or =75 ml/m(2) reflects the risk of subsequent AF, and patients should be closely monitored.

Background and aim of the study: Functional mitral regurgitation (FMR) often complicates dilated cardiomyopathy (DCM), and portends a poor prognosis. Debate over the optimal treatment continues, underscoring the present incomplete understanding of the patho-anatomic mechanisms of this disease. Studies of mitral tenting volume and tenting area, and echocardiographic measures of abnormal apical systolic leaflet geometry have linked mitral leaflet deformation with subvalvular left ventricular (LV) remodeling in chronic ischemic MR. The relative contributions of annular versus subvalvular remodeling in FMR due to DCM are less clear. Here, the validity of 3-D measurement of mitral deformation, tenting volume, as a correlate of MR in DCM, was tested. The ability of annular and subvalvular remodeling to predict mitral deformation was then determined.

Methods: Eight sheep underwent placement of radiopaque markers on the mitral annulus and leaflets. Global LV, annular and subvalvular geometry, as well as mitral tenting height, area and volume were calculated before (Control) and after the development of pacing-induced cardiomyopathy and MR (DCM). Multivariable regression determined which measure of mitral deformation was the best predictor of MR. Regression analysis was also used to find geometric predictors of mitral tenting volume.

Results: In a multivariable analysis, mitral tenting volume was the only independent predictor of severity of MR (r(2) = 0.79, standard error of estimate (SEE) = 0.58). Increased tenting volume correlated best with increased mitral annular septal-lateral diameter (r(2) = 0.67, SEE = 0.72).

Conclusion: The 3-D tenting volume correlates best with severity of FMR. Mitral deformation (increased tenting volume) observed in DCM is predicted by annular dilation, but not by subvalvular LV remodeling. These data support the use of an undersized annuloplasty in DCM complicated by FMR, and may guide the rational design of new therapies for this vexing disease.
